Identifies and provides direct services to homeless individuals and families. Services may include assessment,
information and referral, distribution of needed basic supplies, as well as on-site client supervision.MQ’s: 6

Works in collaboration with physicians, patient services assistants and Clinic Follow-Up Case Manager to provide general
reproductive care services for teens, men and women, including women in perimenopause, menopause and post
menopause.MQ’s: Must be a Licensed Nurse Practitioner, Physician Assistant, or Nurse Midwife. Must have
dependable/insured transportation, valid CA Driver License, and acceptable driving record.Close Date: Until Filled

Benefits:Full time regular, seasonal and limited term employees are eligible for 100% employer paid benefits. Part-time
regular, seasonal and limited term employees, working 20-29 hours per week on a regular basis are eligible for 50%
